Evidence required to support application

What you will need before you apply...

  • ID for claimant and partner (if applicable) 
  • Proof of national insurance number for claimant and partner (for example, national insurance card, old payslip, letters from the department for work and pensions)
  • The most recent 2 months statements for any bank/building society account or savings accounts held by claimant and partner. 
  • Proof of any other savings, capital or investments for claimant and partner. 
  • Proof of any other income for claimant and partner (including any benefits, allowances, pensions, maintenance or money paid in board and lodgings). 
  • Proof of income (wages etc) for claimant and partner: 
    • If you work for an employer:
      • your last 5 pay slips, if you are paid weekly
      • your last 2 pay slips, if you are paid monthly
      • If you have just started work, do not delay applying. We will let you know what information we need. If you can't find your pay slips or they are handwritten you can ask your employer to complete a Certificate of Earned Income
    • If you are self employed
      • your most recent accounts or tax return.

If you cannot get hold of all this information straight away, submit the form and send us anything outstanding as soon as possible. We cannot finalise your application without it and if you do not supply information we have asked for it may stop your claim from being processed.

Applications in person

Please take all the documents that apply to you when you make your claim.

If there are any other documents required the advisor will tell you what you need to provide or we will write to you if there is anything we need to check when we process your claim.

Please make sure you bring originals not copies, we will scan them in with your claim so you don't have to leave your originals behind.
