Benefit Online Form - Set up Information

Step-by-step setup information

The E-Benefit form uses Windows ActiveX browser plug-in technology. This requires a Windows PC running Internet Explorer 6 or 7 or Mozilla Firefox.

When you first view the form webpage, your browser will check to see whether you have the E-form software installed. If not, it will automatically be downloaded (this is a once-only download).

After the E-form plug-in has downloaded, Windows will ask your permission to install it. You will be shown a software certificate confirming the author of the software (CBF Systems Incorporated). See below.



Step 1 - Please click to accept the software. The E-benefit form will then load.

Step 7 - You may now enter your details onto the form

Windows XP, Service Pack 2

The new version of Internet Explorer released with XP, Service Pack 2 and 3, handles ActiveX browser plug-ins slightly differently.

During the download the following message will appear within the new "information bar" feature at the top of the browser:
Online benefit form active x 1
Online Benefit form active x 2

As soon as the above message appears, please click into it. The software will then continue to download.

You will then be shown a code certificate (see below) verifying the author of the software is CBF Systems Incorporated.

Please click on the Install button to continue.
